If you do not have extended health benefits or you are choosing to submit your claim yourself, full payment is required at the time of service.
Extended Health Benefits
If your extended health benefits provider allows electronic claim submission and provides reimbursement directly to the insured, you will be required to pay at the time of service.
Custom Orthotics
We are pleased to deal exclusively with The Orthotic Group for all our custom orthotic. There is $65 physiotherapist fee for the orthotic assessment appointment for custom orthotics and we do require payment in full at the time of order. Once all essential paperwork is received, a billing package is compiled by our administrative staff for you to send to your insurance company for reimbursement.
Unfortunately, at this time, Green Shield and Blue Cross, Extended Health Benefits does not recognize orthotic dispensing by physiotherapists.
Workers Compensation Insurance Board (WSIB) claims
We are pleased to provide rehabilitation services to injured workers. Once a claim is initiated by the employee or employer, it is essential that the injured worker seeks treatment as soon as possible. Billing is submitted electronically on your behalf and payment goes directly to the provider. It is important that you understand that in some instances, claims may be denied and alternative terms of payment (extended health benefits or private pay) will be required.
Motor Vehicle Accident Claims
A motor vehicle accident is a stressful event, no matter the extent of injury or damage. We are here to help you through this process, from the extensive paperwork to the complex nature of the billing. Injuries sustained in an MVA fall into three categories: minor injuries, non-minor injuries as well as catastrophic injuries.
Point of Balance Physiotherapy Ltd is a licenced provided with HCAI, which is the governing body through which assessment, treatment planning and billing are completed on your behalf. These processes are regulated by the Financial Services Commission of Ontario to protect the interests of the insured, the insurer as well as the health provider.
Should you have extended health benefits, billing guidelines stipulate that your EHB coverage must be used prior to billing the insurance company. This will be arranged with you through our billing office.
Cancellation Policy
Your appointment time was reserved for you. We do appreciate that circumstances arise that may require you to cancel and reschedule your appointment. We request that you provide 2 business days notice of cancellation. Failure to do so may result in a cancellation fee of $50.00. Thank you for your understanding.
